Consent Forms

The below consent form was used this season by the ESFA for players attending ESFA international squad trials. Colleagues should use this as a template and customise for their specific activity.

Sanctioning and Permission to Play

Any schools’ football requires ESFA sanction. The ESFA has clarified and updated its policy for sanctioning and the relevant paperwork is below. Please take the time to familiarise yourself with this and with the new paperwork, as the policy takes effect immediately.

Depending on the nature of the planned activity sanction must be sought, either from the ESFA nationally or from the relevant CSFA or DSFA. 

Sanctioning is essential to ensure that: 

– Those taking part are affiliated to the relevant Association; 

– The rules governing the event comply with Football Association (FA) requirements; 

– Activities can legitimately benefit from the use of registered match officials;

Young Match Officials

If you use match officials who are under 18 at your rep football fixtures, please ensure that you send them this letter when you appoint them. It explains what they should do if they have any safeguarding concerns, including being victims themselves. Please remember to personalise this letter for your own Association.
